Regence Evolve Core is a low-cost catastrophic plan with four deductibles to choose from.  This plan features upfront benefits for office visits, outpatient X-ray & lab and preventive care.  It's a plan designed to encourage wellness and is a great option for those looking to reduce monthly premiums.  Evolve Core does not have maternity, prescription or vision benefits, but members will have access to the RegenceRx discount program.  

Before the deductible, members receive:

• Four upfront office visits covered at 100% after a $35 copay
• $200 in upfront X-ray and lab benefits covered at 100%
• Preventive Care - Covered 100%; no deductible or annual limit!

Regence Evolve Plus offers the most benefits, including maternity, prescription and vision care.  There are four deductibles to choose from with the $1000 deductible plan being the only comprehensive option.

 Before the deductible, members receive:

• Four upfront office visits covered at 100% after a $25 copay
• $400 in upfront X-ray and lab benefits covered at 100%
• Prescription drugs ($10 copay for generics!)
• Preventive Care - Covered 100%; no deductible or annual limit!
• Vision Care: Routine Eye Exam & Hardware - covered at 80% to $150 combined maximum PCY

Additional benefits include:

• No Primary Physician required (no referrals necessary!)
• Includes Physician, Hospital Services, Lab and X-Ray,
         Home Health Care, Rehab, and Much More!
