Dental coverage that increases as you regularly visit the dentist.  Watch your annual maximum increase from $750 to $1,500 and your out-of-pocket expenses decrease as your coinsurance levels increase from 80%/60%/30% the first year to 100%/80%/50% the fourth year (preventive/basic/major services).
